Adding and Editing Patients
Adding a New Patient
- Select the person icon (Patients) from the left navigation to open the Patients list.
- Select the Add Patient button in the upper-right corner of the page.
- Fill in the patient form:
Basic Information (required fields marked with *)

Contact Information
- Phone Number — enter in any format; the system normalizes it.
- Email Address — must be a valid email format if provided.
Address
- Street Address, City, State, and ZIP Code — all optional but recommended for letter delivery.
External Identifiers
Enter any external IDs (MRN, patient IDs from other systems, etc.) in the External IDs field. Type an ID and press Enter to add it as a chip. You can add multiple identifiers. These are useful for cross-referencing with your EHR or claims system.
- After filling in the form, select Save (or the submit button at the bottom). The new patient record opens immediately.
Editing Patient Demographics
From within any patient’s chart:
- Select the pencil icon next to the patient’s name in the chart header.
- The edit panel opens on the right side. Update any demographic fields — name, date of birth, sex, address, phone, email, care team, external IDs, or business unit assignment.
- Select Save to apply your changes.
Managing Notes and Alerts
Notes and alerts appear in the Notes and Alerts card in the patient header area.
To add or edit notes and alerts:
- Select the pencil icon in the Notes and Alerts card title.
- The Notes and Alerts edit panel opens on the right side.
- Add or modify alerts — short, high-visibility flags (color-coded by severity) that appear prominently on the chart.
- Add or modify notes — longer free-text entries that provide context for care team members.
- Select Save to apply your changes.
Assigning a Business Unit
If a patient record has no business unit assigned, the patient header shows an Assign Business Unit button. Select it to assign the patient to one or more business units in your organization’s hierarchy.
